GENERAL
Print or typewrite all information requested. First and last names of all entrants in full required.

Only Booster Division Handicapped. All Booster Prize Fees shall remain in Booster Division prize money. Booster teams shall not be eligible to cash in regular division.

No team or individual shall be entitled to a refund after entry blank and fees have been received and definite dates assigned.

ELIGIBILITY
The Iowa State USBC Bowling Association shall accept entries in the Iowa State Tournament only from teams and individuals bowling under their jurisdiction. In addition, a non-resident who is eligible for the state tournament in their state of residency must be a member of an association in the state of Iowa 30 days prior to bowling.

Unmarried Grade and High School Students who have not attained age of eighteen (18) must have written consent of their parents or guardian in order to participate in this state tournament. Said written consent must be on a form approved by the USBC
and must be on file with the tournament association manager at least one (1) week before the bowler is eligible to bowl in tournament competition, unless the student is accompanied by their parent or legal guardian, in which case the parental consent form may be filed up to the time the student starts to bowl.

A bowler is permitted to bowl only once in the five-person, once in the two-person and once in the individual events. The penalty for an infraction of this rule is expulsion from organized bowling, disqualification of the team and forfeiture of entry fees and prize money

AVERAGES
Highest USBC certified averages at end of 2006 - 07 Season, must be
submitted, USBC Rule 319a-3 shall apply.

Individuals bowling in more than one league will have their eligibility in Booster Division based on their highest USBC certified average. Falsification of average will be subject to disqualification.

Entrants will use highest USBC certified League Average based on a minimum of 21 games for the 2006 - 07 season. If no average last year use highest current USBC Certified average, 21 games or more, as of December 1, 2007.

Summer league average will be used if bowler has no winter league average. All others will bowl at scratch. Scratch shall be 210.

Sport bowling averages will not be used.

USBC Rule 319-A-2, 319-D/319-E will not apply.

Any bowler who reports a lower average than required shall forfeit tournament
entry fees and prize winnings or base prize winnings on submitted
average if it is higher.


SUBSTITUTIONS
When a team entered in the Booster Division makes a substitution in
their lineup which raises the team average above 950, the team must bowl
in Open Division.

No entrant may change their scheduled position in the Five-Person event
after lineup has been handed in at the secretarial desk. If a change in lineup
is desired it must be made at the Assn. Mgr. Office no later than 30 minutes
prior to the time said team is scheduled to bowl.

Both members of a Doubles Team must bowl in the order listed on the
entry blank, on the same pair of lanes and at the same time. If unable to
bowl at scheduled time, a written request must be made with the Assn. Mgr.
of the ISUSBCBA at least 24 hours before the scheduled time of bowling.

Substitution for an original A.E. entrant will be permitted, providing an affidavit is signed prior to participation by the original All Events entrant or substitute in either the Five-Person, Two-Person or Individual Events.

Prize fee for the sixth bowler will be accepted up to closing date of entries if unrestricted A.E. eligibility is desired.

USBC Championship Chevrons will be awarded to the Champions in
each event.


IS USBC BA WILL PAY ENTRY FEE TO THE MASTERS TOURNAMENT FOR THE OPEN ALL EVENTS CHAMPION.

ENTRIES CLOSE DECEMBER 1
All entries postmarked the first post office business day following the entry
closing date shall be accepted. Full remittance (prize, bowling and expense
fees) in check or money order must accompany entry blank.


PRIZE DISTRIBUTION
The prize list shall be prepared on the basis of one prize to every five entries
in Team, Doubles, and Singles; All Events, one prize to every twenty entries.

PENALTY FOR RULES VIOLATION

Any person who bowls under an assumed name, bowls under the name of
an entrant scheduled in the tournament but fails to attend, or who is under
suspension, or any team which employs a person herein described, both the
person individually and the team collectively shall forfeit their entry and any
and all rights to claim or hold any trophy or prize awarded in such tournament.
Such person or persons involved are also subject to suspension.
